Guide to Request Procedures for Personal Information Subject to Disclosure
- 1.Contact for "Request for Disclosure, etc.
Requests for Disclosure, etc." should be addressed to the "Complaints and Inquiries Office concerning Personal Information" by mail, enclosing the prescribed request form, necessary documents, and a handling charge.
- 2.Documents to be submitted for "Request for Disclosure, etc.
To make a "Request for Disclosure, etc.," please fill out the request form in (1) above and mail it to us. Please print out and fill out the request form, which will be sent to you upon request. In order to confirm your identity, we will ask you to provide us with approximately two items of personal information registered with us (e.g., your telephone number and date of birth) that can be used to confirm your identity.

- 3. "Request for Disclosure, etc." by proxy
If the person making the "Request for Disclosure, etc." is a proxy, in addition to the documents listed in 2.(1), please enclose a copy of one of the documents listed in 3.(1) below that certifies that you are the proxy, and 3.(2) a copy of the document certifying the proxy's identity.
(1) Documents certifying that you are the representative
- <In the case of a representative authorized by the principal to make a request for disclosure, etc.
- (1) A letter of attorney from the principal (original)
- <(2) If the agent is a legal representative of a minor
- (1) A copy of the family register
- (2) Certificate of residence (with the family name and address)
- (3) My number card
- (4) Other official documents that can confirm the authority of legal representation
- <In the case where the proxy is a legal representative of an adult ward
- (1) Certificate of registered matters regarding guardianship registration, etc.
- (2) Other official documents that can confirm the authority of legal representation
(2) Copies of documents proving the agent's identity
- (1) Driver's license
- (2) Copy of documents proving the representative's identity (i) Driver's license (ii) Passport
- (3) Health insurance card
- (4) Certificate of residence
- (5) My number card
- (6) Basic Resident Registration Card
- Please black out or otherwise process the information after that.
- 4. Fees for "Requests for Disclosure, etc." and Method of Collection
Only in the case of a request for notification of purpose of use or disclosure, the following fee (including the cost of our reply) will be charged per request. Please enclose a postal fixed-sum money order for the following amount when mailing the request documents.
Fee amount: 300 yen - 5. Method of response to "Request for Disclosure, etc.
In principle, we will reply in writing (by sealed envelope) to the address of the person indicated on the request form.
- Personal information obtained in response to a "Request for Disclosure, etc." will be handled only to the extent necessary to respond to the request for disclosure, etc.
- In the following cases, we may not be able to respond to the "Request for Disclosure, etc.". In such cases, we will notify you to that effect and the reason. Please note that a fee will also be charged for non-disclosure.
- (1) When the identity of the person in question or his/her representative cannot be verified.
- (2) When the prescribed application documents are incomplete
- (3) When the subject of the request for disclosure, etc. does not fall under the category of "personal information subject to disclosure" (*)
- (4) When there is a risk of harm to the life, body, property, or other rights or interests of the customer or a third party
- (5) When there is a risk of causing significant hindrance to the proper conduct of our business
- (6) When it would violate laws and regulations.
- Personal information subject to disclosure is personal information that constitutes a systematic collection of information, and for which we have the authority to respond to all requests for disclosure, correction, addition, or deletion of content, suspension of use, elimination, and suspension of provision to a third party. However, if any of the following a) through d) applies, the information does not fall under the category of personal information subject to disclosure.
- a) Information that may harm the life, body, or property of the person concerned or a third party if its existence or nonexistence is revealed.
- b) Information that may encourage or induce illegal or unjust acts by revealing the existence or non-existence of the personal information.
- c) Information that is likely to cause harm to national security, damage the relationship of trust with other countries or international organizations, or cause disadvantages in negotiations with other countries or international organizations, if its existence or non-existence is revealed
- d) Cases in which the prevention, suppression, or investigation of crimes or the maintenance of other public safety and order may be disturbed if the existence or nonexistence of the personal information concerned is revealed.
Requests from the person concerned or his/her representative for notification of the purpose of use, disclosure, correction, addition, deletion, suspension of use, elimination, or suspension of provision to a third party of personal information held by the Company (hereinafter referred to as "Request for Disclosure, etc.") shall be handled in accordance with the following procedures using the request form prescribed by the Company. The Company will respond to your "Request for Disclosure, etc." in accordance with the following procedures.
